New Risk for Sudden Death in Dialysis Patients

A low concentration of homoarginine in the blood seems to be a risk factor for sudden cardiac death and death from heart failure in diabetic patients who require hemodialysis, the researchers found.

Patients with the lowest levels homoarginine were about twice as likely to die suddenly during a study period of four years as those with the highest (HR 2.1, 95% CI 1.2 to 3.9), according to Christiane Drechsler, MD, PhD, University of Würzburg in Germany, and colleagues.

Those with the lowest level was also a three times more likely to die of heart failure (HR 3.3, 95% CI: 1.1 to 10.1), the researchers reported in the August issue of European Journal of Heart Failure.

"Longitudinal Evaluation homoarginine may be useful for monitoring the risk of dialysis patients," they write. "Beyond that, homoarginine may become a new promising target for therapeutic interventions in populations with high incidence of heart failure and sudden cardiac death."

Sudden cardiac death is a major cause of death in maintenance dialysis patients.Although some risk factors for sudden cardiac death in these patients were identified - including poor glycemic control and vitamin D - they do not fully explain the excess mortality.

Previous studies have suggested that homoarginine - a cationic amino acid - can increase the availability of nitric oxide, and an amino acid deficiency may increase the risk of endothelial dysfunction and infarction.

"We thought that endothelial dysfunction may play a crucial role in arrhythmogenesis in patients with renal failure and tested the hypothesis that low blood levels of homoarginine to increase the risk of sudden cardiac death in dialysis patients," Drechsler and colleagues.

They conducted a post hoc analysis of 1244 patients in the German 4D, which evaluated atorvastatin versus placebo in patients with type 2 diabetes who were treated by maintenance hemodialysis. The primary endpoint was a composite of cardiac death, stroke and MI.

The researchers measured the concentrations in blood samples obtained homoarginine base before the start of hemodialysis treatment or drugs. The average concentration was 1.2 mmol / L.

Low homoarginine concentrations were strongly associated with signs of heart failure, including congestive heart failure, left ventricular hypertrophy, and high levels of brain natriuretic peptide.
